Horizon Home Care and Hospice is honored to be designated a 6-Time Top Workplace!

Apply Today to join our award-winning team!

Horizon Home Care and Hospice, headquartered in Milwaukee, WI, is a home care and hospice provider built on a foundation of care that celebrates the human spirit and the desire for security, comfort and compassion.  We make a loving difference in the lives of our patients and employees.  To some, home is a place. To others, it’s a feeling.  To us, it’s whatever you need it to be, wherever you are.  With Horizon, you’re home.

We make it a priority to hire and nurture the very best health care professionals. Our educators are leaders who work collaboratively with leadership to create policy and shape our organization in what it is - top notch! They demonstrate the knowledge, problem solving skills, and patience to mentor our clinical professional and paraprofessional field staff.

Due to agency growth we have two (2) open RN Educator roles within our Education Department.  First, to round out our Education Team we are seeking an individual that is interested in nurse informatics & education as a career.  If you are secretly a technology guru and enjoy implementing the use of technology within your medical practice, then this could be the perfect fit.  However, if nurse informatics is not your passion, but you have prior home care and/or hospice experience; and you wish to take your career to the next level we have an opportunity for you as well.

In either case, if you are a RN ready to share your health care knowledge and experience with others then our unique and important roles will allow you to be responsible for orienting new home care and/or hospice professional staff, conducting in-services, developing orientation and continuing education programs.  These full-time opportunities provide an optimal work schedule - Monday-Friday 8:00am - 4:30pm, with occasional variances and are based in our beautiful Milwaukee Corporate Offices.

Both positions minimally requires a BSN, valid WI RN license, and 3- 5 years of clinical experience.  Due to the nature of these roles, individuals must be computer proficient and possess strong public speaking skills.

Horizon offers you a unique opportunity to give back and make a difference in your community.  In return, we offer a competitive compensation and benefits package including Health, Dental & Vision insurance, Retirements Savings with Employer Match, Paid Time Off, Mileage Reimbursement, and more!

Horizon Home Care and Hospice, Inc. is an equal opportunity and affirmative action employer that does not discriminate in employment and ensures equal employment opportunity for all persons regardless of their race, color, religion, sex, national origin, or status as a qualified individual with a disability or Vietnam era or other protected veteran. To achieve our goal of equal opportunity, we maintain an affirmative action program through which we take good faith efforts to recruit, hire and advance in employment qualified minorities, women, persons with disabilities, Vietnam era veterans, and other protected veterans.
