Horizon Home Care and Hospice, headquartered in Milwaukee WI, is a home care and hospice provider built on a foundation of care that celebrates the human spirit and the desire for security, comfort and compassion.  We make a loving difference in the lives of our patients and employees.  To some, home is a place. To others, it’s a feeling.  To us, it’s whatever you need it to be, wherever you are.  With Horizon, you’re home.

Horizon is one of the largest and most respected home care organizations in southeastern Wisconsin. We are Federally licensed and accredited by the Joint Commission. Our home care patients, predominantly the geriatric population, receive interdisciplinary care of the highest standards. Therefore, we employ only the most ethical and compassionate nursing staff.

Do you have aspirations for leadership, but can't bear the thought of giving up your patients just yet? Now truly experience the best of both worlds!  As Horizon expands our services within Kenosha County we are looking for a motivated supervisor/RN to join our leadership team.  In the RN Clinical Supervisor role you will initially spend 50% of your time supervising, mentoring, and training our extraordinary team of experienced RNs, CNAs, office staff plus 50% time treating patients out in the field.  You will report to the Director of Home Care.  This role gives you the opportunity to be part of our amazing leadership team and grow your supervisory/management career within our growing agency.

We need a motivated Supervisor to ensure that our Kenosha Office consistently treats patients in adherence to Horizon’s policies and Medicare/Medicaid regulations. If you are a self-directed professional who possesses the ability to lead-by-example and believes in the impact a strong supervisor/mentor can have on staff and how that will enrich quality of care for our patients this is the perfect fit for you.  Futhermore, carry your own caseload of patients to keep your skills sharp, while experiencing that patient face-time and firsthand ability to make a difference that inspired you to become a therapist in the first place!

Qualifications include a BSN degree from an accredited School of Nursing, current WI RN license, and 3 years of related experience, preferably home care and/or supervisory experience. Since this position performs joint supervisory visits in the field and must travel throughout Kenosha County, a valid WI driver's license and auto insurance are also required.

Horizon offers you a unique opportunity to give back and make a difference in your community.  In return, we offer a competitive compensation and benefits package including Health, Dental & Vision insurance, Retirements Savings with Employer Match, Paid Time Off, Mileage Reimbursement, and more!
