Join the dynamic Private Duty Team at Horizon Home Care and Hospice, located in Milwaukee, WI as a Case Manager.
The Case Manager role is a unique blend of both in office and field case management and Home Health Aide (CNA) and Personal Care Worker (PCW) supervision. Enjoy the ability to connect with our patients while providing comprehensive care oversight to include initial referral, admission, care development and coordination, and ongoing case management. Assist in mentoring and supervision of private duty home health aides and personal care workers; and enjoy the balance that comes with a full-time, first shift position. This multi-layered position works collaboratively with other team members including the Home Health Aide Supervisor, Home Health Aide Coordinator, Team Liaison and Director to assure the highest level of care is provided.

This ideal first shift positions works Monday-Friday, 40 hours per week; with 1 weekend a month on call with pay. Qualifications include a degree from an accredited School of Nursing (BSN preferred), current WI RN license, and 3 - 5 years of related home care and/or supervisory experience.
