As a MyChoice Family Care contracted CMU, Horizon's Family Care staff emphasize preventative and holistic care. The half office, half field Registered Nurse/Case Manager position provides invaluable support to Milwaukee's growing geriatric, DD, and behavioral health community. Help Milwaukee residents maintain their independence in the home as long as possible. Meanwhile, help yourself maintain an optimal work/life balance.
This full-time position works desirable 1st shift hours Monday through Friday. NO WEEKENDS!!

Requirements include a minimum ADN degree (BSN preferred), RN license to practice in the State of Wisconsin, and at least one year of direct care experience with geriatric, developmentally disabled, or behavioral health population. This position is based out of our Park Place (Northwest Milwaukee) corporate headquarters but also travels on patient visits throughout Milwaukee County. A valid Wisconsin driver's license and auto insurance are mandatory.
