As a RN Case Manager you will provide the kind of nursing care you've always wanted to provide; all while enjoying the flexibility and independence that only home care offers. As part of an interdisciplinary team, you will provide evaluation, hands-on skilled nursing care and be instrumental in development and management of the treatment plan for our patients in the comfort of their own homes.
Horizon is expanding our service area and therefore we have immediate opportunities available within Kenosha County. Our RN Case Managers enjoy a desirable Monday-Friday, 1st shift schedule with NO On-call and NO Weekends!
Qualifications include a degree from an accredited School of Nursing (BSN preferred), a current Registered Nurse license for the State of Wisconsin, and preferably 1-2 years of Med/Surg or home care experience. Must have valid WI drivers license, reliable vehicle, and current auto insurance.
