Horizon Home Care and Hospice, headquartered in Milwaukee WI, is a home care and hospice provider built on a foundation of care that celebrates the human spirit and the desire for security, comfort and compassion. We make a loving difference in the lives of our patients and employees. To some, home is a place. To others, it’s a feeling. To us, it’s whatever you need it to be, wherever you are. With Horizon, you’re home.
Horizon is one of the largest and most respected home care organizations in southeastern Wisconsin. We are Federally licensed and accredited by the Joint Commission. Our home care patients, predominantly the geriatric population, receive interdisciplinary care of the highest standards. Therefore, we employ only the most ethical and compassionate rehab, nursing and support staff.
Do you have aspirations for leadership, but can't bear the thought of giving up your patients? Now truly experience the best of both worlds! As a Rehab Supervisor you will spend 50% of time mentoring our extraordinary team of experienced therapists plus 50% time treating patients out in the field. You will report to the Rehab Manager and assist with program development, certs, therapist performance evaluations, and new staff orientation. Additionally you will supervise our PTAs, including interviewing, training, and corrective action, when appropriate.
We need a motivated Supervisor to ensure that our rehab department consistently treats patients in adherence to Horizon’s policies and Medicare/Medicaid regulations. If you are a self-directed professional who possesses the ability to lead-by-example and believes in the impact a strong supervisor/mentor can have on staff and how that will enrich quality of care for our patients this is the perfect fit for you. Futhermore, carry your own caseload of patients to keep your skills sharp, while experiencing that patient face-time and firsthand ability to make a difference that inspired you to become a therapist in the first place!
Qualifications include a minimum of 4 years of practicing Physical Therapist experience (home care strongly preferred) and the associated WI licensure, plus the appropriate educational background: Graduate a Physical Therapy program from an CAPTE approved curriculum.
Horizon offers you a unique opportunity to give back and make a difference in your community. In return, we offer a competitive compensation and benefits package including Health, Dental & Vision insurance, Retirements Savings with Employer Match, Paid Time Off, Mileage Reimbursement, and more!
