Our rehabilitation family consists of the very best physical therapists, speech language pathologists, occupational therapists, physical therapy assistants, and support staff.
Enjoy the convenience of working close to home (current opening covering Washington County territory) and the satisfaction of helping those in your community. Under the supervision of a licensed Physical Therapist (PT), perform treatment plans to restore clients’ function and prevent disability following disease, illness, injury or loss of a body part. Therapy is provided in the client’s home through an interdisciplinary team approach. Provide services at quality levels consistent with professional standards of practice.
Horizon Home Care and Hospice, a not-for-profit organization and the second largest home care provider in the area, has been a pioneering leader in developing and offering home health services to southeastern Wisconsin for 25 years. Qualifications include a current PTA license to practice in Wisconsin, Graduation from a physical therapist assistant curriculum approved by the CAPTE, and at least 2 years of Clinical experience (health care setting preferred).
