Job Description

Now more than ever, the future of healthcare is Home Care.

Horizon Home Care and Hospice, headquartered in Milwaukee, WI, is growing and expanding into Racine and Kenosha Counties!

We are seeking an experienced, conscientious social worker to join one of Milwaukee's fastest growing not-for-profits. For over 20 years, Horizon's staff has committed to providing the best care to many of the patients and families in the community who need it most.

We all know health care is changing. This part-time position works a desirable Monday-Friday 1st shift schedule with NO ON CALL AND NO WEEKEND requirements.

Qualifications include a Master’s Degree in Social Work + current APSW Licensure for Social Work within the State of Wisconsin. Must have a working knowledge of the principles and practices of social work, awareness of community resources, ability to relate to people of diverse cultural backgrounds, utilization of various treatment modalities, and the ability to work within an integrated team concept. Prefer at least two (2) years experience in a clinical setting. Current WI driver’s license, reliable transportation, and proof of current automobile insurance also required.

Horizon Home Care and Hospice is a proud 5-time Milwaukee Journal Sentinel Top Workplace honoree.  We only hire the best.  Is this where you belong?
