Our inter-disciplinary Hospice team provides the full spectrum of care for our Hospice patients and offers multiple levels of support for their loved ones.  From our free Grief Resource Center to our award-winning Virtual Hospice at Froedtert, to our many hospice enhancement (massage therapy, pet therapy, music therapy, spiritual guidance, etc.), we tirelessly strive to create the most positive hospice experience possible.

Our Hospice Team is currently seeking a LPN Clinical Coordinator to consistently ensures appropriate staffing and coordination for all team patients. Assign cases based on clinical priority, staff availability and expertise required.  Enter, update, and maintain case assignments in the computer daily.  Work with the Intake Department, track and communicate patient hospitalizations to facilitate patients' continuity of care.  Efficiently coordinate RN, LPN coverage for all planned vacations, unexpected time off, PM and weekend visits, nightly and weekend on-call schedule and holiday coverage. Acts as a resource contact for all team communications.  Ensures phone coverage in Team Assistants absence.  Appropriately refers issues to the manager or designee as needed.  Performs additional responsibilities that support departmental efficiency, team functioning and organizational policy.   

Leave your mornings free for school, family or other commitments and work a desirable FULL-TIME position during the hours of 2:00-10:30PM Monday - Friday with NO WEEKENDS and within our beautiful Park Place Corporate Headquarters in Milwaukee, WI.  

Requires LPN licensure in the State of Wisconsin, plus one year experience as appropriate for team clerical functions.  Working knowledge of Home Care computer system with demonstrated keyboarding proficiency preferred.  Excellent communication, organization, prioritization and customer relation skills.
