Coordinates the daily Clinical Team functions from the office to assure effective coverage of all Team client’s. Acts as a resource for field staff, including Therapists, Case Managers, Patient Care Coordinators, Nurses, Social Workers, Chaplains, and Home Health Aides, while functioning as a liaison that represents the team with all internal and external customers. Provides services at quality levels consistent with professional standards of practice

 

This vital position will:

  • Ensure appropriate staffing and coordination for all team patients, and supports the timely processing of all related paperwork. 
  • Acts as a resource for field staff, while functioning as a liaison that represents the team with all internal and external customers.
  • Provides services at quality levels consistent with professional standards of practice.
  • Participates in holiday office coverage rotation.

  • Working knowledge of Home Care computer system with demonstrated keyboarding proficiency preferred.
  • Current/Valid WI Drivers License; plus reliable/insured vehicle
  • Full-time, Monday-Friday 8am-4:30pm
