Clinical Coordinator - Hospice
Coordinates the daily Clinical Team functions from the office to assure effective coverage of all Team client’s. Acts as a resource for field staff, including Therapists, Case Managers, Patient Care Coordinators, Nurses, Social Workers, Chaplains, and Home Health Aides, while functioning as a liaison that represents the team with all internal and external customers. Provides services at quality levels consistent with professional standards of practice
This vital position will:
- Ensure appropriate staffing and coordination for all team patients, and supports the timely processing of all related paperwork.
- Acts as a resource for field staff, while functioning as a liaison that represents the team with all internal and external customers.
- Provides services at quality levels consistent with professional standards of practice.
- Participates in holiday office coverage rotation.
