RN - Intake, Weekend Care Coordinator (Corporate-Based)

Our Intake Department, has an immediate need for a Weekend Registered Nurse - Care Coordinator. This position will focus their efforts specifically coordinating the intake and referral process for our home care and hospice patients over the weekend. Intake is the hub of our Agency, and we need a nurse with the utmost organization and communication skills to help coordinate and process referrals. Be prepared to be challenged every day; and work with an amazing collaborative, fun, and professional team.
taking Monday-Friday off and working every Saturday & Sunday 8am-8pm as a Weekend RN, Care Coordinator in our beautiful Corporate Offices, located within the Park Place Business Park in Milwaukee, WI. Keep your days free for school, family, or whatever else you feel like! You can use your nursing experience to support our patients strictly on weekends and still be eligible for FULL benefits and a competitive salary.
Qualifications include a degree from an Accredited School of Nursing (BSN preferred), a WI RN license, at least two years of related experience, and computer/typing proficiency. Home Care and Hospice experience. A commitment to unparalleled customer service and fitting in with out collaborative culture are essential. Must be adept at communicating with various departments and staff. Must be knowledgeable and able to pull information from different documents in order to create a summary/order of care needed.
