Horizon Home Care and Hospice, located is Milwaukee, WI is seeking a Hospice Patient Care Coordinator (i.e. Case Manager) to perform referral evaluations / admission assessments of patients qualifying for Hospice services. Work a desirable Mon-Fri 1st shift schedule while assuming 24-hour accountability for safe, effective nursing care of assigned patient / family caseload through the utilization of the nursing process, clinical skill, and Interdisciplinary Group approach.
We offer competitive salaries, full benefit package and bonus structure which includes a $10,000 bonus (prorated based on FTE) upon hire and on-going bonus plan for achievement and maintenance of CHPN certification.
Qualifications include a degree from an Accredited School of Nursing, WI Registered Nurse license, and at least one year of previous nursing experience; home care/hospice preferred. HPNA certification as a CHPN preferred.
