Home care is the fastest growing sector of the health care industry, and Horizon Home Care & Hospice, located in Milwaukee, WI, has been one of the largest and most respected home care agencies in Wisconsin for over 25 years. We are federally-licensed, Joint Commission-accredited, and see roughly 1200 patients per day.

We are currently seeking Home Care Admissions RNs within the counties we serve (Milwaukee, Waukesha, and Racine).  As a Home Care Admissions RN you will visit patients in their private residence or wherever their home may be and will assess and document the patients' home care needs and work collaboratively with nursing and rehab staff to ensure seamless admission procedure.

Requirements include Graduate of an Accredited School of Nursing, Wisconsin Registered Nurse License; and at least 1 year of related experience preferred. Because this position travels throughout our service area, you must have a valid driver's license and auto insurance.  Along with top notch clinical skills, we are looking for the right personality. A caring bedside manner, the ability to build rapport, demonstrate empathy and confidence are essential.  Since your charting is done on your Horizon laptop, computer proficiency is crucial. McKesson experience a huge plus!
