Horizon is a leader in the home care and hospice industry; and to support our growing agency, we are currently seeking a RN Clinical Liaison at Columbia St. Mary's - Ozaukee, located in Mequon, WI to respresent Horizon by helping patients and families make a smooth transition into hospice or home care.  This position also includes some travel as needed.

 

The multi-tasking master in this position will assess patients while they are in the hospital, help coordinate their discharge planning, and educate patients and families about our services. Furthermore, the Clinical Liaison will cultivate relationships with hospital physicians, nurses, social workers, and staff. We seek strong performers with charisma and exceptional customer service to serve as "the face of Horizon". An outgoing persona and strong communication skills are essential. Must be a self-starter able to perform with minimal supervision. Work independently, but collaborate with a top notch intake and liaison team to coordinate referrals. 


We are open to a candidate that would be interested in a full OR part-time position.  In either case, this 1st shift opportunity with no on-call or weekend requirements is the perfect opportunity to find optimal work/life balance.  
This position does include travel throughout SE Wisconsin as needed.


Qualifications include a degree from an Accredited School of Nursing (BSN preferred), a WI RN license, and at least one year of related experience.
