Horizon, a not-for-profit Agency, has been southeastern Wisconsin’s employer and home care provider of choice for 25 years. This is a very exciting time at Horizon -- our home care program has achieved phenomenal growth in recent years, and we project that to continue exponentially.  Consequently, we need to bring on a new LPN to help care for our increasing patient census.

We're looking for that extraordinary nurse who possesses top notch clinical skills combined with a warm, positive personality. Due to the unpredictable nature of home care and the necessary adjustments as our Agency expands, the successful Horizon LPN embraces change, rather than resists it, and is highly adaptable. Our needs are for weekend LPN covering Milwaukee, Waukesha, Racine and Kenosha Counties. Our weekend LPNs work 1st shift, 12 hour shifts both Saturday and Sunday.  Eligible for Health, Dental & Vision Insurance, Retirement Savings Plan with employer match, free Fitness Center, Mileage Reimbursement, Tuition Reimbursement & More.

Qualifications include Graduate of an Accredited School of Nursing, a regular LPN license in the State of Wisconsin, and at least one year of LPN experience in an acute care, long term care (Geriatrics), or home care or hospice setting.  Must be computer proficient; McKesson experience ideal! Because this position travels to patient homes, you must have a valid WI driver's license, a reliable vehicle, and current auto insurance.
